    Pre Survey
  • You are well on your way to knowing your status! Before we get started, there are a few things you need to know:

    • Due to high demand and funding limitations, we are prioritizing at-home test requests for individuals residing in the Atlanta metro area who are unable to easily access a PIHC service center. Priority will be given to those living more than 15 miles from our Duluth or Decatur Centers. Thank you for your understanding and cooperation.
    • You must be 18 or older to participate in this program. If you are between the ages of 13 and 17, you are encouraged to recieve a free HIV screening at one of our physical locations.
    • We unfortunately are unable to mail orders to PO boxes. Only residential addresses will be accepted. If your address has an apartment number, please do not forget to include that in your submission.  Contact information, including name, address, phone number, and email address, must be legitimate in order to participate.

    • This program provides free confidential HIV testing meaning that your name and other identifying information will be attached to your results and testing profile. We are a HIPAA compliant agency and will securely store all data internally. Non-identifying information will be shared with direct funders and will not be shared with insurance companies, non-authorized health care providers, or any other 3rd party entities without written consent.
    • We encourage clients to recognize the CDC standard “window period” which explains that a negative HIV result accurately represents an individual’s status as it pertains to 90 days prior to receiving that result. Possible HIV exposures that have taken place within the last 90 days may not accurately be represented in your result.

    • Each participant can receive 1 free At-Home HIV kit every 90 days. If you have recently participated in this program and would like to be tested for HIV, you are encouraged to visit one of our physical locations to receive that service.
